The situation

BrightSmile is a busy private practice with three dentists and two hygienists. Their practice management system handled clinical records and the diary well, but patient communication was a single generic text the evening before, and it didn't ask for a reply. Their no-show rate over the previous six months was 23%.

The front desk compensated by phone. Every morning two people spent the first hour and a half calling the day's patients, leaving voicemails, and rebooking gaps by hand. Hygiene recalls were sent as letters twice a year, and nobody had time to chase the ones that didn't respond.

Thomas Reid, the practice manager, wanted two things: fewer empty chairs and his team's mornings back. He was also clear that anything touching patient data had to be reviewed by their compliance lead before it went live.

What was leaking

One generic reminder with no confirm option. Confirmations done by phone every morning. Cancellations leaving gaps nobody could fill in time. Recalls by letter, unchased. Weekend enquiries waiting until Monday.

Same order as every engagement: the CRM set up right, then the automations that move the numbers, then everything around them. Each step was tested with real test leads before the client's customers saw it.

    Map the patient journey and the data boundary

    A single diagram from first enquiry to recall, agreed with Thomas and the compliance lead. We defined exactly which fields would live in GoHighLevel (name, contact details, appointment type and time, consent) and which would never leave the practice system.

    Reminders that get an answer

    Three touches: seven days before by email, 48 hours by SMS with a one-tap confirm or reschedule, and three hours by SMS. Replies update the appointment status automatically, so the front desk sees a list of unconfirmed patients instead of a list of everyone.

    Waitlist and rebooking

    A cancellation or reschedule triggers an offer to matching waitlisted patients within minutes, first come first served. A no-show receives a warm rebooking message the same afternoon with a link, and the front desk is only involved if nobody rebooks.

    Recall campaigns in stages

    Patients overdue for hygiene or check-ups were pulled from the practice system export, segmented by how long overdue, and invited in weekly batches so the diary could absorb the bookings. Non-responders got one follow-up, then a final message.

    Enquiry response and reviews

    Web forms and missed calls now get a friendly reply and a booking link instantly, including weekends. Completed visits trigger a thank-you and review request; anything negative routes to Thomas rather than Google.

    Compliance review, testing and launch

    The compliance lead signed off the data flow document. We ran test patients through every reminder and recall branch, checked opt-out handling, and trained the front desk with a recorded walkthrough and a laminated one-pager.
